Help Logging into Patient Portal
For security reasons, passwords should be changed regularly. If you are having trouble accessing your health information, please follow these guides or call our office at 1-817-240-2343
Steps for Creating a Portal Login Account in 4 easy steps:

Click on the green Patient Portal button and you will get the main log in screen.

Click Create account on the LEFT side on the screen. You will be taken to the next screen. Enter the information that you gave us when you first visited the office. If you are unsure or think any information has changed, call the office at (817) 240-2343 to verify

On the next screen, verify that the information is correct and that you entered a phone number for which YOU HAVE IMMEDIATE ACCESS. Once you click continue, you will receive a phone call within 90 seconds that will speak to you a code that you must type into the next screen.

Fill out a password that meets the requirements listed on the right side of the page and filll out the three required security questions.
To sign up with your PIN:

Click on the green Patient Portal button and you will get the main log in screen.

Click sign in under the green box on the right side. You will be taken to the next screen. Enter the 5-digit PIN that you gave us when you first visited the office. If you are unsure or never set up a online portal PIN, please call the office at (817) 240-2343 and ask the receptionist to look up your account. At this time you can give her the 5 digit PIN number that you would like to use for your portal login.
